Life Insurance With No Medical Exam in Illinois: Your Options

Dusty Bruggeman, Licensed Insurance BrokerApril 12, 20265 min read

Nobody loves the idea of a nurse showing up at their house to draw blood. The good news: many life insurance policies in Illinois no longer require a medical exam. The tradeoff? You'll usually pay a bit more.

Here's how to decide whether a no-exam policy makes sense for you, what the three types actually are, and roughly what each option costs.

The Three Types of No-Exam Policies

“No medical exam” covers three very different products. Knowing which one you qualify for matters, because the price and coverage are dramatically different.

1. Accelerated Underwriting

This is the best of both worlds. You apply, the carrier runs your information through electronic health databases (MIB, prescription history, DMV, public records), and if everything checks out, you're approved in days — without a single needle. Coverage amounts up to $1 million or more are available. Rates are the same as fully underwritten policies.

The catch: you need to be relatively healthy to qualify. If the automated systems flag anything unusual, you'll either get kicked to a traditional underwriting process or offered a simplified issue policy instead.

2. Simplified Issue

Simplified issue policies ask a short list of health questions (usually 5–15) but skip the exam. Coverage is typically capped at $50,000 to $400,000 depending on the carrier. Rates are higher than fully underwritten policies — figure 15–40% more for the same coverage — but the process is fast and straightforward.

This is a solid middle ground for people with manageable health conditions, older applicants, or anyone who just needs coverage in place quickly (closing on a home, new baby, travel plans).

3. Guaranteed Issue

Guaranteed issue policies accept everyone, regardless of health. No questions, no exam, no way to be turned down.

The tradeoffs: coverage is limited (usually $5,000 to $25,000), premiums are the highest per dollar of coverage, and there's a two- to three-year graded death benefit period. If you die of natural causes during the waiting period, beneficiaries get premiums back plus interest instead of the full death benefit. After the waiting period, the full benefit kicks in for any cause of death.

These policies are designed primarily for final expense and burial coverage — specifically for people who have been declined elsewhere or have serious health conditions.

Who Should Skip the Exam?

If you're in good health and qualify for accelerated underwriting, absolutely take it. You get the same rates as a fully underwritten policy without the hassle of scheduling a paramedical visit.

If you have manageable health conditions — well-controlled diabetes, a family history of heart disease but clean personal labs, mild sleep apnea — or you simply need coverage fast, simplified issue is a solid option. You might pay 20% more, but you'll be approved in a week instead of six.

If you've been declined by other carriers or have serious conditions that make traditional underwriting a non-starter, guaranteed issue ensures you can at least get something in place.

What It Actually Costs

Ballpark ranges for a non-tobacco Illinois applicant in decent health:

  • $500,000 accelerated underwriting 20-year term at age 35: $25–$35/month
  • $100,000 simplified issue 20-year term at age 45: $40–$60/month
  • $15,000 guaranteed issue whole life at age 65: $80–$120/month

These are rough ranges. Your actual rate depends on your specific profile and the carrier placed. The only way to know for sure is to run your profile through multiple carriers.

How to Avoid the Classic Mistake

The biggest mistake I see is people assuming they need guaranteed issue because they don't want the exam — then paying way more than they should.

If you can get accelerated underwriting at a standard rate, that's your move 9 times out of 10. Don't pay the simplified-issue premium unless you actually need to skip the questions or you've been declined before.
